On being accepted for finance we will send you a consumer credit agreement which you will need to sign and return prior to surgery. On the day of treatment you will need to bring Photo ID (passport, driving licence or national identity card only), a copy of your latest bank statement from which you will be making payments and your completed standing order form from your bank.
